Referring a Case for Rehabilitation Services

We are pleased to accept referrals in a manner most convenient to you. 

If you wish to discuss specific issues relating to a file before making a referral or to enquire about our service capabilities, please contact our, Quality Assurance and Practice Standards Manager at 416-923-5059.

5 Ways to Make a Referral:

1. Call our Head Office at 416-923-5059 and indicate that you wish to refer a file. We will confirm the nature of the services required.
   
   Once you have made the referral by telephone, we ask that you forward any file material that our consultant will need to understand the particulars of the file.
2. Fax your referral to our Head Office at 416-923-2126. For your convenience the Rehabilitation Referral Form in PDF format can be downloaded and printed here.

4. Online Referrals:
   When D. Gary Gibson & Associates is in receipt of your referral we will e-mail acknowledgement of the assignment, assign the file immediately and provide written confirmation of the consultant assigned to the file within 24 hours. Click here to submit a referral online.
5. Case discussion is available with a specific consultant. A direct referral through this consultant can be accommodated.

With all referrals, we assign our consultant and issue a confirmation note within 24 hours. The consultant will contact you, usually within 24 hours of receipt of the referral.
